Although we hear a little bit about your life from the professionals around you, we very much like to hear you tell your version of your story. Your story is at the centre of your time with us and is used to help us to understand how you would like to be supported by us and what emotional needs you might have. Your support plan outlines any goals, aspirations and support needs that you might have and helps us to aim towards these with you during your time with us.
Key Events and day to day life:
We want to capture all of the amazing moments that happen in your life and also the times when things are a bit more difficult, so we can work through them together. Every part of your journey is important to us.
RISK ASSESSMENT:
This document identifies potential risks and provide sa supportive framework that helps you stay safe and secure. They are living documents that can be updated and adjusted as needed, ensuring they remain relevant to your evolving situation.
My emotional regulation plan
When we experience difficulties growing up it can affect the way that we regulate our emotions. This plan helps us to work together to recognise what is happening when things are difficult for you and develop strategies and ways of relating with each other that support you to feel emotionally contained and manage what is happening.
